From the Desk of Dr. Stephen Mulholland: Why Medspas Don’t Need More Devices—They Need Better Systems
Every week, I meet medspa owners who believe the fastest path to growth is purchasing another device. A new laser. A new body contouring platform. A new skin-tightening technology. The salesperson convinces them that this one investment will “transform” their practice, bring in new patients, and elevate their brand.
But after 25 years in this industry, I can tell you: devices don’t build empires—systems do.
A treatment device is a tool. But a system is an engine. And engines outperform tools every time.
The most successful medspas I’ve seen are often not the ones with the most expensive technology. They are the ones with:
- intelligent marketing infrastructure
- real-time lead conversion
- consistent consultations
- automated workflows
- efficient scheduling
- strong retention systems
- predictable revenue forecasting
In fact, I’ve seen clinics with a single injector outperform clinics with a full suite of devices simply because they operated on superior systems.
Here’s the truth device reps won’t tell you: devices have diminishing returns without the foundational systems to support them. If your clinic cannot handle leads efficiently, cannot convert consultations effectively, and cannot retain patients predictably, no device will save you.

Before buying a device, ask yourself:

- Is my consultation conversion rate above 60%?
- Do I have automated reminders, workflows, and follow-ups?
- Is my marketing driven by AI optimization?
- Is my team fully leveraged or drowning in admin tasks?
If the answer to any of these is “no,” you don’t have a device problem—you have an operational problem.
Devices amplify systems.
Systems amplify revenue.
If you strengthen the system, every device becomes more profitable. If you ignore the system, every device becomes a burden.
Growth doesn’t come from adding technologies. Growth comes from installing the operating system that makes every part of your clinic perform at its highest possible level.
